What is Adult Foster Care and who can qualify?

What are the steps of the Adult Foster Care application process?

The adult foster care application process involves an assessment of the applicant’s needs, a review of their eligibility for the program and a home study of the potential care provider’s residence. Background checks and fire safety inspections may also be required. After selecting an AFC provider, the next step is to submit an application and complete the assessment process. 

Medical Evaluation 

A healthcare professional must confirm that the person needing care meets MassHealth’s medical criteria for Adult Foster Care. A social worker or care manager will assess the applicant’s functional abilities and identify their need for assistance with daily living activities. 

Home Assessment 

The AFC provider will visit the caregiver’s home to ensure it’s a safe and suitable environment for long-term care. You’ll also need to gather personal, medical, and financial information, including proof of identity and residency.

  • AFC PCP Order Form: This form, completed by the member’s Primary Care Provider, verifies the member’s medical necessity for AFC services. 
  • Personal and Medical Information: You’ll need to provide details about your health, medical conditions, and functional limitations that require assistance with Activities of Daily Living (ADLs). 
  • Financial Information: Proof of MassHealth eligibility is required, as AFC is a MassHealth program. 
  • Identity and Residency Documentation: You’ll need to provide documents to prove your identity and residency, such as a driver’s license, passport, or utility bills.

The care team will work together to develop a personalized care plan, defining the specific services and support the applicant will receive.
