With more than a decade of combined experience in the field of home care, the Mass Care Link leadership team channeled their learnings into a new type of home care organization, based on inclusivity and compassion. By working with families and providers to develop custom care plans, we ensure each member receives the best care possible. Our collaborative care plans educate and empower caregivers to improve outcomes and enhance quality of life.
Ongoing Monitoring & Support
Coaching, training and guidance helps members and their caregivers stay abreast of safety requirements and standards of care while building the skills and confidence they need to succeed. Our network offers support and comfort for caregivers to prevent burnout and build community.
Faster Payment & Reduced Cost
Ensuring members receive the payment from MassHealth promptly while managing the cost of care helps families from underserved communities maintain the dignity and quality of life accessible through home-based care.
Refer a Patient
A referral to Mass Care Link for Adult Foster Care can allow elder patients or adults with disabilities to receive care at home from someone they trust. If you have seen people in your practice who could benefit from home care, we can help them receive the assistance they need.
Who is Eligible?
To qualify for the Adult Foster Care program, your patient must be a Massachusetts resident and elderly or disabled (and over the age of 16). Applicants must also require assistance with at least one of their activities of daily living on a day-to-day basis, with a doctor’s prescription confirming this is the case. While they require daily care, they may not have a medical condition that requires hospitalization or a residential facility. In addition to the medical requirements, program participants must be financially eligible for MassHealth.
