Please remember the following for your home visit:
- Complete your Daily Log of Care Journal prior to the visit. If you need help, view instructions here.
- The member and caregiver need to present at the home visit.
- The home visits need to be conducted at the home.
Cancellation Policy: In order to better serve you, all scheduled home visits must be kept as planned. Cancelling a scheduled home visit may affect your monthly payment from MassHealth. If you must cancel an appointment, please call in advance and speak to the person assigned to your case. Messages left over the weekend are not considered sufficient notice. Proper documentation for your cancellation is required in order to be able to schedule a new appointment.
